Many patients thinking about a breast augmentation ask whether they should do an under or over the muscle augmentation. There is no simple "one size fits all" solution to this question, as all patients have unique situations. Typically, a brief one on one consultation can determine the right path to accomplishing your specific goals, but there are some techniques that have produced consistent results for breast augmentations over the years.
In theory, an over the muscle breast augmentation (under the gland) would produce better results than the under the muscle augmentation. This is because we are augmenting the breast, not the entire chest. Typically, this is not the case though. The over the muscle breast augmentation is more visible, creating more of an unnatural round arc at the top of the breast which will stick out of the bra or dress. Placing the implant under the muscle tends to create more of a gradual transition from the collar bone to the nipple, as opposed to placing the implant over the muscle which looks more like a "bump" on your chest.
For a first time breast augmentation, a textured silicone implant placed under the muscle has been an extremely successful procedure. A textured implant simply means that it does not have a smooth outer shell (you can think of the texture as a comfortable fabric chair you have sat on). This implant is placed under the pectoralis major muscle, and on top of the pectoralis minor muscle creating a pocket that will grab on to the texture keeping the implant in place. This produces breasts that position themselves as natural, well shaped breasts. If the same first time breast augmentation were done using smooth implants, they may slip around a bit more in the pocket allowing gravity to have more of an affect which will give you more of a bottom heavy breast. If the patient came back to have a breast revision done after the pocket has been created, a smooth implant works very well.
